US Immigration Medical Exam Requirements

For a USCIS Certified Civil Surgeon to complete the physical exam form (Form I-693) and clear you for the purpose of obtaining a U.S. permanent residency (Green Card) you will need the following:

1. A physical examination or a medical check-up

2. Bring all your vaccine records and blood test results for vaccination related antibodies when you come in. f you have received all the vaccines but do not have proof of the vaccination, you can opt for the blood test to check if you have protective antibodies from a prior vaccination in your blood.

3. Bring your passports or drivers license or a government issued photo ID

Note: If you have health insurance blood tests may be covered under your plan. We do provide vaccines but the rates vary from time to time. You have the option to get the vaccinations from a Pharmacy.
